    All children and ADULTS coming onto the premises must have a completed waiver. Each adult must have a completely separate form.

  • I, , understand that there is no insurance coverage for my child/children who will be participating in various activities with Crystal Peaks Youth Ranch in 20 *. This includes, but is not limited to, community service projects, hauling hay, participation in parades, special outings (some overnight), and gatherings at restaurants. All will be planned and overseen by staff members from Crystal Peaks Youth Ranch.

    I am fully aware of the risks involved in the above said activity and under no duress, hereby do release Crystal Peaks Youth Ranch Co, its Board of Directors, founders, and staff of all Liability either expressed or implied regarding the welfare of my child/children including liability for any injury or death caused by negligence but not grossly negligent or intentional misconduct. I accept full responsibility for the health and safety of the child/children indicated above, through my own oversight and personal insurance coverage for any/all accidents, injury, duress that may or may not occur at this event.

  • Equine Activity Release and Hold Harmless Agreement

  • 1. I, , the undersigned have read and understand, and freely and voluntarily enter into this Release and Hold Harmless Agreement with the property owners (“Stable”) of 19344 INNES MARKET RD, BEND, OR 97703 (the “Property”), understanding that this Release and Hold Harmless Agreement is a release and waiver of any and all liability(ies) against them and against the property, for all claims resulting in injury or death including negligence but not grossly negligent or intentional misconduct.

    2. I understand the potential dangers that could incur in mounting, riding, walking, boarding, and feeding horses; including, but not limited to, any interactions with horses. Understanding those risks I hereby release Stable, its officers, directors, shareholders, employees and anyone else directly or indirectly connected with Stable from any liability whatsoever in the event of injury, damage of any nature, or death to me or anyone else caused by or incidental to my electing to approach, feed, mount, care for, and/or ride a horse at the Property. I understand these equine activities and events are voluntary in nature and are not essential or necessary to further any public policy.

    3. I understand and recognize and warrant that this Release and Hold Harmless Agreement is being voluntarily and intentionally signed and agreed to, and that in signing this Release and Hold Harmless Agreement I know and understand that it may further limit the liability of Stable, and any equine professionals on the Property, to include any activity, whatsoever, involving an equine, including death, personal injury and/or damage to property.

    4. I recognize and agree that I know the people who are helping and helped me engage and come into contact with horses on the Property, and acknowledge that I agree such person(s) has/have made reasonable and prudent efforts to determine my ability to engage in the equine activity, and has/have sufficient knowledge of my equine and horseback riding skills as to relieve, release and hold harmless said persons from (1.) any continuing duty to monitor my equine activities, and (2.) claims of negligence, but not grossly negligent or intentional misconduct.
